Umoja HAP
The Umoja Health Access Point (Umoja HAP) is San Francisco’s first one-stop harm prevention and community health and wellness hub for Black communities. Created by the people for the people, the center provides holistic health services, education, and culturally-appropriate care to ensure that every person receives the support they need to thrive.
Why Umoja HAP
We believe in a San Francisco where Black communities have a fair and just opportunity to reach their best health and wellbeing, have spaces to care for their minds, bodies, and spirits, and can feel safe and supported.
While only making up 5 percent of the city’s population, the Black community in San Francisco faces some of the highest health disparities that continue to be exacerbated by systemic inequities. Umoja Health Access Point aims to bridge this gap by centering the impact of structural racism and addressing the impact that social issues – lack of housing, access to family sustaining wage jobs, and equitable education – continue to have on the health and wellness of Black communities. By offering a variety of services that focus on addressing underlying inequities and prevention, we strive to create a healthier, more resilient community.
Through our work, we hope to:
- Get to zero new HIV infections, and zero HIV-related deaths
- Eliminate HCV
- Reverse increasing STI rates
- Eliminate racial disparities in access to services and health outcomes
Our Services
Umoja HAP provides comprehensive behavioral and sexual health services to San Francisco’s Black communities, along with access to fresh food, testing, counseling, and support groups. Additionally, given the high rates of overdose deaths and ongoing housing affordability issues within our community, our services address the systemic care needs and what’s needed to build an ecosystem of care for impacted individuals and their families.
Mental Health Services
Comprehensive counseling and therapy, addressing the emotional and psychological needs of our community members.
STI Testing
Confidential and free testing services for sexually transmitted infections, and promoting sexual health and well-being, including free condoms and contraceptives for all.
HIV & AIDS Support
Specialized support and resources for those living with HIV/AIDS, and helping individuals manage their health and well-being.
Overdose Prevention
Educational training, narcan resources, and holistic interventions to prevent drug overdoses.
Holistic Wraparound
A holistic approach to health care that offers integrated services to address physical, emotional, and social well-being.
Food Access
Access to nutritious, organic food and support for food security.
Family Planning Support
Contraception, prevention counseling, prenatal care, and educational resources to empower individuals and families in making informed reproductive health choices.

UCSF Alliance Health Project
Partnering to provide essential public health testing services and resources to our community.
3rd Street Youth Clinic
Offering mental health support and age appropriate information and resources tailored to the needs of the Black community for transitional age youth.
SF AIDS Foundation (SFAF)
Providing comprehensive services and support for those living with HIV/AIDS.
Positive Resource Center (PRC)
Promoting workforce, housing support, education, and community development in the Bayview Hunters Point neighborhood.
