San Francisco’s first one-stop harm prevention and community health and wellness hub for Black and historically marginalized communities.

What is Umoja HAP?

Umoja HAP is a groundbreaking initiative designed to provide comprehensive health and wellness services specifically created for and by the Black community. By bringing essential services together in one location, Umoja HAP works to address health disparities and remove barriers to care while ensuring services are delivered with dignity, cultural responsiveness, and compassion.

How to Access Services

Walk In

Stop by during walk-in hours. No appointment is required for many services.

Call Ahead

Call 415-615-9945 to speak with a team member or schedule an appointment.

Get Referred

Ask your case manager, healthcare provider, or a community organization to refer you.
