Health Screenings
We collaborate with other community-based organizations to provide health screenings. This serves as the first step in helping people understand and take control of their health.
Rafiki Wellness Passport Program
Passport is a culturally affirming and wellness-centered program designed to assist participants who want support in reaching and maintaining a healthy lifestyle. Participants commit to achieving personal health goals through a holistic program of lasting changes.
- They set personal goals and attend health education classes, nutrition workshops and exercise classes.
- They take advantage of complementary and alternative medicine services.
- They receive ongoing physical and mental health support.
- They make new friends who are working on their own health goals.
We conduct two or more programs a year.
Rafiki Patient/Wellness Navigator Program
We provide Patient/Wellness Navigators to people who need help navigating a very complex health care system. Navigators coach participants in developing health action plans and then assist them, as needed, in:
-
- Finding the appropriate care
- Scheduling and getting them to their appointments
- Reminding them about their medications
Navigators are your Rafiki friend in health and wellness. Our Wellness Navigation team, Maxine and RoShon continue to be available for your queries. Please feel free to contact RoShon at 415-349-0799 and Maxine at 510-680-3277.
Rafiki First Saturday Health Party
Our monthly First Saturday Health Party is a neighborhood party that is good for the mind, the body and the soul. It’s a great way to participate and learn about all the programs and services we offer. All are welcome — meet new people, exercise, have fun, and eat a delicious healthy lunch.
Complementary Medicine Clinic
Complementary medicine can be beneficial to a person’s overall well-being. Rafiki’s approach focuses on helping heal the mind, body and spirit. Our practitioners use treatment methods, such as massage and acupuncture.
